Although this incident does not involve Delta Dental of New Jersey or Connecticut, Change Healthcare is a vendor we used and we are providing this information to help our members who might be affected.

| For employer-sponsored claims Please mail your claims to the below address: Delta Dental of New Jersey P.O. Box 16354 Little Rock, AR 72231 Most members won’t need to do anything since our participating dentists in New Jersey and Connecticut have already been notified of the change. Members who reside out of state or see an out-of-network dentist should inform their dental office of the change to reduce any possible delay in their claims. Members can also print an updated ID card with our new address through their MySmile account |
